Honor Your Heroes
At CHCM, we know that giving is personal. With more than one in three Minnesotans affected by chronic illness, it’s likely that someone you know is dealing with chronic disease either as a patient or a caregiver.
You can give a donation in honor, or in memory, of somebody who has changed your life.
We encourage you to make your gift to CHCM in honor or in memoriam of a loved one. We would like to help recognize your friend who is caring for a parent with Alzheimer’s or remember a relative who has succumbed to cancer. CHCM will notify the person you choose to say thanks for inspiring someone to give.

Health Champions
CHCM Health Champions are donors that give a gift of $500 or more.
As a CHCM Health Champion, you have the opportunity to support a wealth of health research, services and educational programs from our 33 member health charities. A Health Champion gift supports vital programming, from the cutting-edge genetic research at Children’s Cancer Research Fund to assistive equipment rental at the ALS Association, Minnesota Chapter. As a Health Champion, you can help improve lives affected by chronic illness.
