Use the following documents to complete the campaign process and ensure that CHCM receives all necessary information.

Your Client Services representative will assist throughout the wrap-up process.

• Wrap-Up/Remittance Form (word document): Use this form to report final results to CHCM.
• Tally Forms for Large Organizations (word document): Use this worksheet to subtotal results by department, floor, building, or other unit before completing the wrap up/remittance form. 
• Company Gift Form (word document): Complete this form to provide documentation for a donation to CHCM from your company.
• Donor Thank-You Letter (word document): Use this modifiable template to send a thank you note or email to employee donors.
• Campaign Recognition: Your commitment to giving sets an outstanding example for organizations across Minnesota and deserves to be recognized! CHCM presents campaign recognition awards to our campaign partners - let us know about the hard work you've done!
